| List of documents required for new Postpaid & Prepaid connections |
Documents for proof of identity (attested copy of any of the following):
- Income-Tax PAN Card
- Photo Credit Card1
- Photo Identity Card issued by State/Central Government
- Passport
- Arms License2
- Driving License
- Election Commission ID Card
- CGHS/Medical Insurance Card3
- Certificate of identity along with attested photograph and signature of the customer duly singed by either a Member of Parliament or a Member of Legislative Assembly or a Municipal Councilor or a Gazetted Officer on a personal letterhead of the certifier and documentary ID proof of the signing authority enclosed with such a certificate – original to be attached
- Photo identity card issued by Govt. recognized educational institutions (for students only).
Documents for proof of address (attested copy of any of the following):
- Ration Card
- Election Commission ID Card
- Water bill
- Telephone bill4
- Electricity bill5
- Income Tax Assessment Orderj
- Vehicle registration certificate
- Applicant’s Passport
- Arms License
- Driving License
- CGHS/Medical Insurance Card6
- Certificate of address issued by Bank Manager/Village Panchayat Head/ Local Post Master7(in original) for rural area only.
- Certificate of identity along with attested photograph and signature of the customer duly singed by either a Member of Parliament or a Member of Legislative Assembly or a Municipal Councilor or a Gazetted Officer on a personal letterhead of the certifier and documentary ID proof of the signing authority enclosed with such a certificate – original to be attached

1. Such cards issued by private banks / organisations should not be accepted as a proof of identity.
2. To be accepted as proof of identity only if it contains a photograph.
3. Insurance card issued by a State-owned insurance company only shall be accepted.
4. Telephone bill issued by a State-owned service provider only to be accepted for this purpose.
5. Such bills issued by a private company shall not be accepted for this purpose.
6. Insurance card issued by a State-owned insurance company only shall be accepted.
7. To be accepted only if the certificate contains attested photograph and signature of the applicant and is issued on personal letter head of the certifier along with copy of ID/ address proof of the certifier.
